Please carefully read and review the following information before completing the Registration Form

As the legal parent/guardian or participant, you recognize the risks of physical injury inherent to dance and dance training and fully assume those risks. You agree to release and hold harmless Kingdom Dance Collective, LLC, its owners, operators, teachers and employees, from any and all liability from injuries sustained or illnesses contracted while attending or participating in dance classes, rehearsals, workshops, or performances. This includes claims, demands, and causes of action whatsoever, arising out of or related to any loss, damage, or injury, including death, that may be sustained by the participant and or parent/guardian, while in or upon the premises or any premises under the control and supervision of Kingdom Dance Collective, LLC, its owners, operators, teachers and employees, or in route to or from any of said premises. You also agree to indemnify, defend, and hold harmless Kingdom Dance Collective, LLC, its owners, operators, teachers and employees for liabilities, costs, and judgements arising from acts of omissions committed by you or the participant which result in any damage to any person or property.

Kingdom Dance Collective, LLC reserves the right to use photographs and videos taken of participants during classes, rehearsals, and performances for advertising and promoting of Kingdom Dance Collective, LLC programs. This includes use of photos and videos on the website, social media platforms, and public media. Students or parents/guardians of students who are minors that do not wish to comply to this policy must make Kingdom Dance Collective, LLC aware prior to participating in class.

When you enroll, you grant permission to Kingdom Dance Collective, LLC, its owners and operators, to seek medical treatment for the participant in the event they are not able to reach a parent or guardian. You also authorize Kingdom Dance Collective, LLC, in the case of a physical injury or medical emergency, to make necessary arrangements to transport your child to a medical treatment facility if necessary. All such transportation and medical treatment will be at the sole cost and expense of the parent/guardian or participant.

Tuition is due by the 7th of each month.
Payments can be made through the website, cash or check in person, or by check mailed to our mailing address. *NOTE: Payments made online will be charged an online processing fee.
A late fee of $25 will be added for any payment that is not made by the 7th.
A $25 fee will be added to any returned checks.
If payment is late, dancers will not be allowed to participate in classes until payment is made and can only return to class once payment, including late fee, is paid.
Kingdom Dance Collective holds the right to ask a dancer to withdraw from classes if late payment reoccurs for multiple months.
There are no make-ups, refunds or credits given for any classes that are missed due to illness, injury, or school conflicts. *NOTE: This applies only to The Collective classes. Company and Production classes have an added policy that is given if and when dancer passes the audition.

Kingdom Dance Collective, LLC is a faith-based Company and reserves the right to speak freely and openly about matters and beliefs of the Christian Faith. By agreeing to this policy, you are agreeing to allow your child to be taught Christian and Biblical values and engage in prayer, worship, and open discussion about Christianity.
